Job Summary

This Physical Therapist position is in Physical Medicine & Rehabilitation service at the VA Connecticut Healthcare facility, located at the West Haven, CT location. This position is a Full-time at 40 hours per week.

Major Duties

The Physical Therapist at the full performance level demonstrates the ability to make autonomous clinical decisions in a Direct Access environment including the ability to independently evaluate and treat patients who seek physical therapy services. The Physical Therapist performs examinations and evaluations of individuals who have or may develop impairments, activity limitations, and participation restrictions related to conditions of the musculoskeletal, neuromuscular, cardiovascular, pulmonary, and/or integumentary systems while considering the effects attributable to unique psychosocial and environmental factors. The Physical Therapist interprets findings from examination and evaluation, select appropriate tests and measures, and integrate findings into the physical therapy plan of care for the full range of patient populations.Duties include, but are not limited to: Interpret findings from examination, diagnostic studies/medical tests and, during evaluation, select appropriate clinical tests and measures, and then integrate findings into the physical therapy plan of care for the full range of patient populations. Establish a diagnosis within the scope of physical therapy and identify the appropriate intervention to treat and/or refer to another provider for further consultation when needed. Determine physical therapy prognosis by incorporating examination findings with the patient's preferences for meaningful level of function in order to set clinically appropriate treatment goals and optimize outcomes. Participates in PM&RS in services and provides in services to PM&RS and other facility staff as assigned by supervisor or Chief, PM&RS. Documents evaluations, goals, treatment plans, and rehabilitation potential, using standard format in patient's medical record as required by PM&R, The Joint Commission, and VA CT Healthcare System standards. Ensures that subordinate Physical Therapy staff and trainees maintain appropriate documentation. Participates in Performance Improvement monitoring including the formulation of monitors, data collections, peer reviews, and preparation of reports in accordance with VA and Joint Commission guidelines.
