Job Summary

The VA Central Iowa Healthcare System is recruiting for (1) full-time Advanced Practice Nurse or Physician Assistant in Rehabilitation and Extended Care (REC) Pain Management Service to provide a full range of services emphasizing quality, timeliness, and the advancement of the Veterans Affairs (VA) mission. This announcement is for Physician Assistants applicants.

Major Duties

The VA Midwest Health Care Network advocates for a Whole Health System of care in each of the Medical Centers. This is an approach to healthcare that empowers and equips people to take charge of their health and well-being and live their lives to the fullest. As an employee operating in a Whole Health System of care, you will operate in a model with three core elements, seeking to create a personalized health plan for each Veteran. This is done in the context of healing relationships and healing environments and a connection back to the Veteran's community. This aligns with the Veterans Health Administration (VHA) Mission Statement to Honor America's Veterans by providing exceptional health care that improves their health and well-being. Major duties and responsibilities of the position: Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice. Develops and implements an individualized plan to achieve optimal population health and disease management. Demonstrates leadership and works collaboratively with the interdisciplinary team. Orders and interprets labs, tests, and diagnostic studies. Prescribes pharmacologic and non-pharmacologic interventions. Initiates referrals and consults to specialty services. Provides education to patients and their families. Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes. Exemplifies the ICARE core values of-Integrity, Commitment, Advocacy, Respect, and Excellence. Facilitates collaboration of the interdisciplinary staff members located throughout the medical center, promoting a team approach for the coordination of care. Completes detailed history and physical exam, orders emergent and screening diagnostic tests, reviews diagnostic test results with communication of results to Veterans and/or families. Completes discharge summary and medication reconciliation in timely fashion consistent with service line guidelines. Completes documentation of clinic encounters in timely manner consistent with service line guidelines. Evaluation and management of symptoms and functional status, Care coordination/recommendations, education and counseling about goals of care and advanced care planning. Participates in interdisciplinary patient care conferences, assisting in the development of appropriate individualized and goal-oriented plans of care. Initiate consults to other services, including specialty departments as appropriate. Document assessment process accurately, clearly, and concisely when developing initial and ongoing plans for patient care. Provides education and counseling to patients and families regarding preventive care, medical conditions, and use of prescribed treatments and drugs. Consistently makes sound judgments based on accurate and thorough assessments. Consults and collaborates with other health team members. Prescribes medications as appropriate for patients and will follow local and VHA guide- Work Schedule: 7:00am - 3:30pm.
