Diagnose and manage acute and chronic medical conditions specific to older adults. Develop individualized care plans in collaboration with patients, families, and interdisciplinary teams. Review, reconcile, and optimize medication regimens to minimize polypharmacy and adverse drug events. Provide consultative or primary outpatient, inpatient, and/or long-term care to geriatric patients. Assess and manage neurocognitive disorders, including dementia, delirium, and depression.
Coordinate patient care with other healthcare providers, specialists, therapists, pharmacists, and social workers. Evaluate and address functional decline, falls, mobility issues, and frailty in elderly patients. Facilitate referrals to specialty services, community agencies, rehabilitation programs, and support resources. Conduct discussions regarding prognosis, goals of care, advance care planning, and advance directives with patients and families.
Provide education and counseling to patients and caregivers on disease management and preventive care. Document all patient encounters and care plans accurately and timely in the electronic health record (EHR). Participate in interdisciplinary team meetings, case conferences, and quality improvement initiatives. Ensure compliance with facility policies, procedures, and accreditation and regulatory requirements.
Provide teaching and supervision to medical students, residents, fellows, and other healthcare staff regarding geriatric medicine. Engage in continuing medical education and stay current on best practices and guidelines in geriatric care. Participate in research and scholarly activities as appropriate. Perform additional clinical and administrative duties as assigned by leadership. Work Schedule: M-F, 8-4:30pm Bargaining Unit: This position is a AFGE Bargaining Unit position.
Degree of Doctor of Medicine or an equivalent degree resulting from a course of education in allopathic medicine or osteopathic medicine. The degree must have been obtained from an institution whose accreditation was in place for the year in which the course of study was completed. Approved schools are: Schools of medicine accredited by the Liaison Committee on Medical Education (LCME) for the year in which the degree was granted.
Schools of osteopathic medicine approved by the Commission on Osteopathic College Accreditation (COCA) for the year in which the degree was granted. For foreign medical graduates not covered in (1) or (2) above, confirmation must be made that the medical school meets (or met) Educational Commission for Foreign Medical Graduates (ECFMG) eligibility requirement for year graduated.
NOTE: The Under Secretary of Health or designee in the VHA Central Office may approve the appointment under authority of 38 U.S.C. 7405 of a physician graduate of a school of medicine not covered above if the candidate is to be assigned to a research, academic, or administrative position with no patient care responsibilities.
The appointment will be made only in exceptional circumstances where the candidate's credentials clearly demonstrate high professional attainment or expertise in the specialty area.
