Geriatrics and Extended Care (GEC) at the Greater Los Angeles Veterans Administration Healthcare System (GLAVAHS) has two available full-time Home Based Primary Care (HBPC) Physician positions. The candidates should be board-certified in Internal Medicine or Family Practice. As a staff physician in GEC, you will provide comprehensive care to adult and older adult patients.
The HBPC program provides interdisciplinary, comprehensive health care services to Veterans in their home. HBPC patients are often older adults with multiple chronic medical conditions for whom travel outside the home to access clinic-based care may be difficult. The HBPC clinical staff drive government vehicles to the homes of patients to provide them with primary care in the home setting. Some patient care may be provided virtually (e.g. via telephone or video visits).
The positions are based primarily at either the Sepulveda VA Medical Center or the West Los Angeles VA Medical Center, but the positions will also require seeing patients outside the Sepulveda or WLA HBPC catchment areas and cross-covering for other GLA HBPC providers. Excellent clinical and interpersonal skills are required.
The candidates will participate in the instruction and supervision of VA-UCLA Geriatric fellows as well as medical students and residents rotating through HBPC. The candidates should demonstrate expertise in the areas of adult primary care and management of chronic medical conditions. The candidates should be fellowship-trained in geriatrics and/or have significant clinical experience in geriatrics.
The candidates are expected to qualify for faculty appointment at the Assistant Professor level or higher at the University of California at Los Angeles (UCLA). Identifying a patient's medical problems. Defining the medical management of a patient's medical problems. Determining the need for consultation from medical, surgical, psychiatric, or other subspecialty clinics. Determining the need for and facilitating admission to the hospital and other transitions of care. Visiting the HBPC patients at home for medical evaluation and treatment. Must also be able to conduct, as appropriate, virtual appointments using VA-approved platforms.
Providing medical administrative support such as completing physician-required orders and forms for the HBPC patients. Planning and directing the educational and clinical experience of medical students, residents, and fellows assigned to the HBPC Program. Assuming a leadership role in the development and implementation of HBPC's process for continuous performance improvement. Being readily available to the team members for collaboration when clinical or program issues arise.
Participating in the development of the individual plan of care at interdisciplinary care meetings. Work Schedule: Monday - Friday, 8:00 am-4:30 pm
Degree of Doctor of Medicine or an equivalent degree resulting from a course of education in allopathic medicine or osteopathic medicine. The degree must have been obtained from an institution whose accreditation was in place for the year in which the course of study was completed. Approved schools are: Schools of medicine accredited by the Liaison Committee on Medical Education (LCME) for the year in which the degree was granted.
Schools of osteopathic medicine approved by the Commission on Osteopathic College Accreditation (COCA) for the year in which the degree was granted. For foreign medical graduates not covered in (1) or (2) above, confirmation must be made that the medical school meets (or met) Educational Commission for Foreign Medical Graduates (ECFMG) eligibility requirement for year graduated.
NOTE: The Under Secretary of Health or designee in the VHA Central Office may approve the appointment under authority of 38 U.S.C. 7405 of a physician graduate of a school of medicine not covered above if the candidate is to be assigned to a research, academic, or administrative position with no patient care responsibilities.
The appointment will be made only in exceptional circumstances where the candidate's credentials clearly demonstrate high professional attainment or expertise in the specialty area.
