The Nebraska Western Iowa VA-Lincoln, NE Community Based Outreach Clinic (CBOC) is recruiting for full time, permanent Primary Care Physician. The Physician will be responsible for delivery of primary care to patients enrolled at the VA Nebraska-Western Iowa Health Care System. The provider will manage scheduled and unscheduled patients in the outpatient clinic setting. The primary care provider is an integral member of the Patient Aligned Care Team (PACT) teamlet, similar to a medical home team, and is expected to work closely and coordinate patient care with other PACT teamlet members including nursing, clerical pharmacy, social worker, dietitian, etc. Primary care physicians will participate in daily huddles with other teamlet staff and participate in primary care systems redesign projects. A primary care provider in the outpatient setting is assigned a panel of patients to manage under Primary Care Management Model (PCMM) guidelines; recommended panel size at the division level is calculated based on patient complexity, support staff and room availability. Each provider's panel size is specific to their practice and setting. Primary Care Physicians may have responsibility for supervision of or collaboration with primary care mid-level providers. Physicians are expected to provide clinical oversight and to participate in peer review of other primary care providers through periodic chart reviews. Primary care physicians will act as surrogates for absent primary care providers when needed, to include review of CPRS view alerts. Primary care physicians will regularly attend primary care staff meetings, departmental meetings and medical staff meetings. Timely documentation (notes and encounter forms) is necessary in all areas in accordance with facility policy. All administrative work should be completed in a timely fashion. All physicians will work within their privileges at all times and follow the Medial Bylaws. Degree of Doctor of Medicine or an equivalent degree resulting from a course of education in allopathic medicine or osteopathic medicine. The degree must have been obtained from an institution whose accreditation was in place for the year in which the course of study was completed. Approved schools are: Schools of medicine accredited by the Liaison Committee on Medical Education (LCME) for the year in which the degree was granted. Schools of osteopathic medicine approved by the Commission on Osteopathic College Accreditation (COCA) for the year in which the degree was granted. For foreign medical graduates not covered in (1) or (2) above, confirmation must be made that the medical school meets (or met) Educational Commission for Foreign Medical Graduates (ECFMG) eligibility requirement for year graduated. NOTE: The Under Secretary of Health or designee in the VHA Central Office may approve the appointment under authority of 38 U.S.C. 7405 of a physician graduate of a school of medicine not covered above if the candidate is to be assigned to a research, academic, or administrative position with no patient care responsibilities.
