VISN 10 is seeking a Physician of a Patient Aligned Care Team (PACT) (physician, RN, LPN, and clerical associate) which provides direct care for Veterans enrolled in Primary Care at the Hamilton CBOC.

Duties: Provide clinical examination, diagnosis, and treatment of acute and chronic diseases, preventive care, and coordination of care for a panel of primary care patients. Appropriate documentation of patient care in clinical records is expected. The incumbent refers to specialists when additional input and treatment outside of the scope of the physician's experience and training is required.
The incumbent is responsible for monitoring PACT teamlet performance to assess adherence to VA standards, assure quality and effectiveness of care, and meet VA performance measures. Outstanding customer service is expected. The incumbent leads the team in assisting the patient advocate and administration in resolving patient concerns and inquiries. The Primary Care Physician is assigned responsibility for a panel of patients at a specific location.
The incumbent may be asked on some occasions to assist in patient care or administrative duties at the main hospital or at another site. The incumbent should have competence in the effective teaching, monitoring and coordinating of educational activities including planning, evaluating and documenting. The incumbent may have the opportunity to serve as an attending physician on the inpatient service which includes teaching residents, fellows or other trainees.
The incumbent may also have administrative responsibilities, including program planning or leadership, committee involvement, and other augmenting duties for the medical center, and making administrative decisions that affect primary care programs. The incumbent must have excellent communication skills and relations with staff and community and is able to elicit cooperation from others.
This includes interacting in person, by phone, email, residency program software, and via other electronic platforms. The incumbent serves as a member of a larger multidisciplinary treatment team which includes social work, pharmacy, mental health, nutrition, and on occasion, other services. Degree of Doctor of Medicine or an equivalent degree resulting from a course of education in allopathic medicine or osteopathic medicine. The degree must have been obtained from an institution whose accreditation was in place for the year in which the course of study was completed. Approved schools are: Schools of medicine accredited by the Liaison Committee on Medical Education (LCME) for the year in which the degree was granted.
Schools of osteopathic medicine approved by the Commission on Osteopathic College Accreditation (COCA) for the year in which the degree was granted. For foreign medical graduates not covered in (1) or (2) above, confirmation must be made that the medical school meets (or met) Educational Commission for Foreign Medical Graduates (ECFMG) eligibility requirement for year graduated.
NOTE: The Under Secretary of Health or designee in the VHA Central Office may approve the appointment under authority of 38 U.S.C. 7405 of a physician graduate of a school of medicine not covered above if the candidate is to be assigned to a research, academic, or administrative position with no patient care responsibilities.
The appointment will be made only in exceptional circumstances where the candidate's credentials clearly demonstrate high professional attainment or expertise in the specialty area.
