Provide primary and acute care services to include chronic disease management and preventive health care to patients across the housing spectrum, including care in the home, community housing programs, and outpatient clinic settings. Demonstrate clinical competence in history taking, examination, diagnosis, therapeutic ability, effectiveness in emergencies, patient management consultations, and record keeping.
Provide same day care for patients and provide coverage for other HPACT providers when needed. Participate in Process Improvement/Quality Assurance projects and other administrative duties as assigned. Support the VA mission through participation in applicable VA national, VISN, and local quality and process improvement initiatives, including performance measures and monitors, and systems redesign/advanced clinical access projects.
Work with supervisory and frontline staff in his/her own and other service lines to foster and improve the operation of the section and related system wide initiatives and priorities. Communicate with and utilize other disciplines, including nursing, Medicine, and social work, to facilitate treatment planning and implementation.
Collaborate with a variety of community agencies and engage in problem resolution activities as it pertains to primary care practice for unhoused Veterans All other duties as assigned. Work Schedule: Monday-Friday 8:00am-4:30pm
Degree of Doctor of Medicine or an equivalent degree resulting from a course of education in allopathic medicine or osteopathic medicine. The degree must have been obtained from an institution whose accreditation was in place for the year in which the course of study was completed. Approved schools are: Schools of medicine accredited by the Liaison Committee on Medical Education (LCME) for the year in which the degree was granted.
Schools of osteopathic medicine approved by the Commission on Osteopathic College Accreditation (COCA) for the year in which the degree was granted. For foreign medical graduates not covered in (1) or (2) above, confirmation must be made that the medical school meets (or met) Educational Commission for Foreign Medical Graduates (ECFMG) eligibility requirement for year graduated.
NOTE: The Under Secretary of Health or designee in the VHA Central Office may approve the appointment under authority of 38 U.S.C. 7405 of a physician graduate of a school of medicine not covered above if the candidate is to be assigned to a research, academic, or administrative position with no patient care responsibilities.
The appointment will be made only in exceptional circumstances where the candidate's credentials clearly demonstrate high professional attainment or expertise in the specialty area.
