The Lead LPN at the GS 7 level must exercise independent judgment and requires minimal supervisory oversight. Assesses, records, and reports vital signs, neurological signs, weight, and blood glucose levels. Responsibilities include prompt recognition and treatment of urgent/emergent patient care needs, including hypo/hyperglycemia, acute shortness of breath, acute chest pain, seizures, stroke symptoms, mental health issues, etc.
Maintains and implements knowledge of anatomy and physiology as well as scientific principles underlying nursing care and procedures. Provides high quality nursing care using traditional and non-traditional modalities, including triage prior to provider face-to-face visits, nurse health maintenance visits, telephone triage, secure messaging, etc. Serves as a resource for complex/specialized procedures (i.e.
non-complex suture/staple removal, non-complex wound care, blood draws, and clinic specific procedures, such as ear irrigation).Performs venipuncture for blood draws, prepares specimens for transport and processing (centrifuge and serum separation), transcribes orders, accessions labels, provides patient education about lab tests, etc.
Demonstrates knowledge of standard precautions, medical asepsis, and sterile technique with the ability to correctly apply these principles and techniques and assist others with implementation in the clinic setting. Assists with diagnostic procedures that include preparing the patient, setting up sterile field and instrumentation, and monitoring the patient's condition.
Duties include, but may not be limited to the following: Prepares, administers, and documents medications and immunizations as directed without error that may cause hard to patients. Demonstrates practical and theoretical knowledge about medication side effects, purposes, therapeutic dosage range, and toxic effects with skill sufficient to administer a wide range of medications.
Demonstrates advanced knowledge and skill in preparation, administration, and documentation of commonly prescribed oral, topical, subcutaneous, and intramuscular medications permitted by facility policies and procedures and within the LPN Scope of Practice. Recognizes subtle signs and symptoms of drug interactions and promptly notifies provider.
Prepares and administers prescribed medications (oral, topical, subcutaneous, intramuscular and/or intravenous) and performs treatments according to established policies/procedures. Identifies medication deviations from normal and collaborates with provider to consider modifying patient medication regimen. Collaborates with RN, provider, and/or pharmacist to address patient medication questions and/or changes in patient conditions from prescribed medications/treatment.
Performs basic nursing care duties such as taking vital signs, taking samples, and passing instruments. Initiates an emergency response appropriate to the situation and provides resuscitation efforts independently. Communicate verbally and in writing to adequately convey the current clinical status of any patient effectively and promptly.
