Job Summary

The Program Specialist serves as the Primary Care Management Module (PCMM) Program Manager for the General and Specialty Primary Care Teams within New Mexico VHA Healthcare Services and is operationally assigned to the Ambulatory Care Service Line. The incumbent works under the direct supervision of the ACOS of Ambulatory Care. This individual is responsible for maintaining the PCMM database for all of Primary Care throughout the hospital service area and the Community Based Outpatient Clinics.

Major Duties

Duties include, but may not be limited to the following: Provide assistance to managers in performing analyses and/or evaluation of operating program characteristics. Troubleshoot and coordinate processes with all providers and clerks involved with PCMM. Provide expertise and advice on an on-going basis to all providers and clerks. Conduct portion of organizational surveys in a wide variety of organizational activities. Analyze workflow to determine potential problem areas and makes recommendations to correct these areas. Perform clerical work in support of the care and treatment given to patient. Provide overall administrative management of PCMM. Manage administrative functions associated with the PCMM Primary Care program for the facility. Maintain accurate enrollment data. Identifies and corrects discrepancies in database system. Generate the following reports: panel sizes, team assignments, enrollment capacity, current capacity, etc. when requested by administration. Apply basic data gathering methods, such as standard interviewing or surveying techniques to collect various types of factual information.
