The position is located in the Geriatrics & Extended Care Service, Home Based Primary Care (HBPC). HBPC is an interdisciplinary team with a physician, nurse practitioners, registered nurses, social workers, pharmacists, psychologist, dieticians, and rehab therapists who provide primary medical and mental health services in the home eligible homebound veterans.
Major duties include but are not limited to: The incumbent performs work involving the collection, compilation, and/or tracking of data and statistical information in support of an organization's program operation. Generates and reviews documents, reports, and/or applications for omissions and inconsistencies, and ensures data entry is complete and accurate.
Performs and completes special research projects related to the acquisition and/or development of program information and resource materials/personnel to support the policy development and/or technical activities of the organization. The incumbent is specifically involved in projects, such as, Chart Audit Process, Performance Improvement Projects (for local HBPC and VISN HBPC collaborative projects), Infection Surveillance and Patient Satisfaction Surveys.
Refers to and coordinates with other VAMC programs when appropriate serves as HBPC's Service Level Representative to Patient Advocate office. Performs other related duties as assigned.
