Job Summary

RN is responsible and accountable for all elements of the nursing process when providing and/or supervising patient care. Assesses ,plans, implements, and evaluates care based on age-specific components. Assumes responsibility for the oversight and coordination of care for patient referrals within the Veterans Health Administration (VHA), focused on patient education, self-management, and customer satisfaction throughout the continuum of care.

Key Responsibilities

The Nurse Admission/Discharge Coordinator Registered Nurse (RN) is responsible for providing competent, evidence-based care to assigned patients and oversight of licensed vocational nurses/nursing assistants, as appropriate to the setting. The nurse is an active member of an interdisciplinary team whose primary focus is on care transitions to and from an inpatient setting.

Assists with patient planning assessments, identification of barriers, and activities required to successfully transition the patient in and out of the acute inpatient setting. Applies the nursing process which includes systematic data gathering, assessment, and appropriate nursing judgment to ensure positive patient outcomes. Coordinates and collaborates with other VA and non-VA departments, community agencies, and healthcare providers to ensure patient needs are addressed.

The individual will assist in directing the provision of nursing education, orientation, competencies and providing quality improvement and outcomes utilization consultation. Demonstrates leadership in delivering and improving holistic care through collaborative strategies with others. Evaluates practice in an ongoing process, based on best evidence. Provides peers with informal constructive feedback for improvement.

Fosters a safe and supportive environment conducive to the professional development of healthcare professionals. Contributes professional nursing perspective in discussions with the interdisciplinary team. Partners with others to effect change and produce optimal outcomes. Supports colleagues through knowledge sharing to provide safe, quality nursing care. Shares educational findings, experiences, and ideas with peers.

Questions clinical practices for the purpose of providing evidence-based care. Participates in activities and strategies to sustain an evidence-based-practice culture.

Preferred Experience: -BSN -Utilization management experience (1-2) -Case Management/Discharge planning experience (2-3years-Acute Nursing Experience (3 years or greater) -Charge Nurse (1-2 years) -Knowledge of Durable Medical Equipment -Experience in interdisciplinary communication -Ability to demonstrate initiative, be self-directed, and work in collaboration with personnel of all hospital services. Work Schedule: Tour 8 or 10 hour shifts.
