The Caregiver Support Program (CSP) Clinical Assessor Registered Nurse (RN) supports caregiver programs by ensuring compliance, quality care, coordinated services, and providing education to Veterans and their caregivers. The role conducts assessments and case management, collaborates with interdisciplinary teams, and supports program improvement across the Program of Comprehensive Assistance for Family Caregivers.

VA offers a comprehensive total rewards package: VA Nurse Total Rewards Duties of the RN Caregiver Support Program Clinical Assessor include, but are not limited to: Conducts comprehensive clinical assessments for Veterans and caregivers in PCAFC and PGCSS. Performs home, facility, telehealth, and phone assessments to determine program eligibility and care needs. Coordinates care plans emphasizing patient safety, education, and self-management.
Provides referrals and consults to VA and community services as appropriate. Ensures compliance with program standards, documentation requirements, and regulatory guidelines. Collaborates with interdisciplinary teams to optimize care outcomes and customer satisfaction. Participates in program-level quality improvement initiatives and evidence-based practice activities.
Uses critical thinking and sound judgment to match Veteran needs with appropriate caregiver support services. Supports VISN initiatives related to CSP, including MISSION Act implementation and program expansion. Offers constructive peer feedback and contribute to a supportive, professional team environment.
