The Registered Nurse (Case Management) position is within the Nursing, Acute Care service at the Providence VA Healthcare system, located in Providence, Rhode Island. It is permanent and full time at 40 hours per week
The Case Management RN provides patient management, care coordination, and discharge/disposition planning for inpatient and outpatient settings.
Duties include, but are not limited to: Assists Veterans, family members, and caregivers with receiving the most appropriate options and services to meet their complex health care needs; this includes, but is not limited to, acute, chronic, multiple, complex, catastrophic, or life-threatening illnesses; combat stress, residuals of traumatic brain injury; community adjustment; addictions and other health problems; Coordinates care with multiple providers across all levels and sites of care; Addresses psychosocial, as well as nursing and medical needs of patients and their families/caregivers, through participation in interdisciplinary patient care management practice; Evaluates care and outcomes to ensure timely and appropriate provision of services; Establishes a therapeutic relationship, allowing the patient to attain, maintain or regain optimal function through assessment and treatment; Engages patients, families, and other caregivers to incorporate knowledge, values, and beliefs into care planning without judgement or discrimination; and Performs other duties as assigned.
