The LTC Charles S. Kettles VA Medical Center is seeking a Full-Time Registered Nurse (RN) - Care Manager Care Coordinator, aligned under Chief of Staff
The Outpatient Staff Registered Nurse (RN) is responsible for providing competent, evidence-based outpatient care to assigned patients with oversight of licensed vocational nurses/nursing assistants as appropriate to the setting. Assumes responsibility for the coordination of care focused on patient transition through the continuum of patient care, patient and family education, patient self-management after discharge, and supporting factors that impact customer satisfaction.
The nurse will assist in directing the provision of nursing education, orientation, competencies and providing quality improvement and outcomes utilization. Practice includes directing other providers. Partners with the patient, family, and interdisciplinary team to promote effective and safe transitions across the health care continuum.
This nurse has fundamental outpatient experience with the situations in which they are ultimately expected to perform, integrating theoretical concepts, basic critical thinking skills, and technically skilled practice.
