The Registered Nurse will assume responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care.

Responsible for coding the section of the Resident Assessment Instrument (RAI)/MDS Assessment and the Care Area Assessments (CAAs) that are assigned to nursing, adhering to current Centers for Medicare and Medicaid Services (CMS) and Veterans Health Administration (VHA) timeframes of assessment completion. Responsible for the completion of assigned nursing sections in the RAI/MDS Assessment.
Participate in the CLC Interdisciplinary Team care plan meetings and reviewing, communicating, and monitoring the residents' care plan. Collaborate with the CLC Resident Assessment Coordinator (RAC) to educate CLC staff on the completion of the RAI/MDS process; this includes but is not limited to documentation requirements for accurate MDS data entry, educational sessions pertaining to RAI/MDS software and program updates as needed.
Support quality improvement activities based on CLC quality management data. Participate in ongoing survey readiness activities in the CLC. Document and take corrective action when notified by the CLC management team, charge nurse, or designee of any discrepancies in coding or documentation of RAI/MDS assessments and CAAs reported during the interrater reliability review. Bring principles and theory with limited application of critical thinking skills.
