The Iowa City VA Medical Center is recruiting for a Registered Nurse - Community Care Coordinator. The Registered Nurse (RN) Community Care (CC) Coordinator is responsible for executing a streamlined approach to receiving, triaging, and directing timely, coordinated care.
The VA Midwest Health Care Network advocates for a Whole Health System of care in each of the Medical Centers. This is an approach to healthcare that empowers and equips people to take charge of their health and well-being and live their lives to the fullest. As an employee operating in a Whole Health System of care, you will operate in a model with three core elements, seeking to create a personalized health plan for each Veteran.
This is done in the context of healing relationships and healing environments and a connection back to the Veteran's community. This aligns with the Veterans Health Administration (VHA) Mission Statement to Honor America's Veterans by providing exceptional health care that improves their health and well-being. Major duties include, but are not limited to: Provides and supervises safe, high-quality direct patient care using all elements of the nursing process.
Assesses, plans, implements, and evaluates Veteran care across the continuum of services. Coordinates timely and efficient care, supporting access to both VA and community-based services. Serves as a liaison between Veterans, VA providers, and community providers to ensure seamless care coordination. Triages Requests for Services (RFS) and develops comprehensive care coordination plans.
Reviews community provider medical documentation and communicates care needs throughout the episode of care. Applies knowledge of VA referral processes, clinical review criteria, utilization management standards, and community care regulations. Uses systems theory and nursing judgment to organize, facilitate, and guide Veterans through all available care options.
Ensures compliance with VA-mandated processes and appropriate use of health information systems for documentation and care tracking. Collaborates with interdisciplinary teams to influence care outcomes and support high-quality, evidence-based practice. Promotes patient education, self-management, customer satisfaction, and timely access to care. Shares professional nursing insights, supports colleagues, and fosters a safe, development-focused environment for healthcare staff.
Participates in unit and program-level quality improvement initiatives and customer service efforts. Questions and evaluates clinical practices to support an evidence-based practice culture.
