Job Summary

The Community Living Center (CLC) Restorative Coordinator Registered Nurse (RN) is responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care at both the Lake City and Gainesville, FL CLCs.

Major Duties

The Community Living Center (CLC) Restorative Coordinator Registered Nurse (RN) is responsible for providing competent, evidence-based care to assigned residents and oversight of licensed vocational nurses/nursing assistants, as appropriate to the setting. The nurse will assist in directing the provision of nursing education, orientation, competencies, and providing quality improvement and outcomes utilization consultation. Functions or scope of assigned duties include, but are not limited to: Coordinates individual restorative care plans for all residents that qualify for the restorative care program. Accountable for Restorative Program training for all nursing staff in the CLC. Coordinates with interdisciplinary team members (OT, PT, educators, therapy services) to establish restorative goals and treatment plans. Demonstrates leadership and advanced clinical judgment in managing complex restorative resident care. Serves as a role model, mentor, coach, and resource for nursing and restorative personnel. Participates in or leads team meetings, committees, councils, and performance improvement activities. Responsible for auditing the restorative documentation process and providing real-time feedback to nursing staff. 8. Compile data to support process improvement and report preparation. Provides support to and participates in the survey process. Evaluates patient progress toward their goals monthly and institutes changes in therapy, goals, and activity when necessary. Completes the restorative nursing sections of the Minimum Data Set (MDS), specifically the observation period, quarterly reassessments, and significant changes in care. Monitors assignments to ensure they are appropriate to the level of education and training demonstrated by restorative personnel.
