Job Summary

Cheyenne Veterans Affairs Healthcare System (CVAHCS) is a Level 3 facility with service-lines serving medical, mental health, surgical, and neurological outpatients, with a Level 3 Emergency Department, and multiple Community-Based Outpatient Clinics (CBOCs). There is a large outpatient psychiatric population, with admissions for medical co-morbidities. Home-Based Primary Care is provided in the veteran's home.

Key Responsibilities

Community Living Center (CLC) Registered Nurse (RN) is responsible for providing competent, evidence-based care to residents with oversight of licensed vocational nurses/nursing assistants as appropriate to the setting. The nurse will assist in directing the provision of nursing education, orientation, competencies and providing quality improvement and enhance outcomes utilization.

This nurse integrates knowledge, skills, abilities, and judgment and is self-directed in goal setting for managing complex situations. Validates understanding of the rationale for care processes and decisions. Questions clinical practices for the purpose of providing evidence-based care. The nurse will demonstrate the ability to cope with and manage competing priorities.

The nurse demonstrates competence in practice and decision-making, deliberate planning, and critical thinking skills. The CLC RN collaborates with the Minimum Data Set (MDS) Coordinator to ensure accuracy of the Resource Utilization Group(RUG) score. The Registered Nurse (RN) in this role is located in the Community Living Center (CLC) of the CHYVAMC.

This person performs the following roles: Charge Nurse, Medication Administrator and provider of clinical treatment for veterans on the unit. The incumbent follows provider orders and is competent to start and maintain Intravenous therapy. As Charge Nurse, maintains the daily flow of the unit, creates and issues daily reports, verifies orders, communicates with providers, families and visitors.

The RN is expected to assign and supervise the work of licensed practical nurses and certified nurse aides to ensure the flow of the unit meets the clinical requirements and is performed safely to meet quality standards. The CLC RN is responsible for ensuring artifacts of cultural change promote residents rights and a home like environment.

The RN has assigned collateral responsibilities which may include infection control, wound monitoring and reporting, staffing methodology, Inter-Disciplinary Treatment Team (IDTT) meetings, restorative, reassessment tracking, and or safe patient handling.
