Job Summary

Eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. Must meet specific eligibility requirements per VHA policy and submit EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Major Duties

A Community Living Center (CLC) is a Department of Veterans Affairs (VA) Nursing Home. Veterans may stay for a short time or, in rare instances, for the rest of their life. It is a place where Veterans can receive nursing home level of care, which includes help with activities of daily living and skilled nursing and medical care. The Nurse Community Living Center (CLC) Registered Nurse (RN) executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex resident care. The Community Living Center Registered Nurse duties include, but are not limited to, the following: Collaborates in the development, implementation, evaluation, and revision of policies, procedures, and/or guidelines. Demonstrates performance and leadership that is broad enough to improve the care for a group of residents. Provides peers with formal or informal constructive feedback for improvement. Supports colleagues and other nurses through knowledge sharing to provide safe, quality nursing care. Fosters a safe and supportive environment conducive to the professional development of health care professionals. Evaluates outcomes of evidence-based decisions and practice changes for individuals, groups, and populations. Utilizes resources appropriately, to support maximization and fiscal responsibility. Responsible for the documented outcomes at the program or service level. The CLC RN collaborates with the Minimum Data Set (MDS) coordinator to ensure accuracy of the Resource Utilization Group (RUG) score. Documents care according to policies and procedures, and Resident Assessment Instrument/Minimum Data Set (RAI/MDS). Performs comprehensive physical, psychological and social assessments; detects early clinical changes in the patient's condition; prioritizes actual and potential problems and needs. Provides resident centered care based on CLC guidelines and principles of practice.
