Job Summary

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (1-5 years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application.

Key Responsibilities

This Registered Nurse (RN) position is located at the HJ Heinz Campus within the Community Living Center (CLC) in Pittsburgh Pennsylvania. The incumbent is responsible to the assigned rating and approving supervisors and is accountable to the Associate Director of Patient Care Services (ADPCS).

The Community Living Center Registered Nurse is responsible for providing competent, evidence-based care to assigned residents and oversight of licensed vocational nurses/nursing assistants, as appropriate to the setting. The nurse will assist in directing the provision of nursing education, orientation, competencies and providing quality improvement and outcomes utilization consultation.

Demonstrates leadership in delivering and improving holistic care through collaborative strategies with others. Provides peers with informal constructive feedback for improvement. Fosters a safe and supportive environment conducive to the professional development of healthcare professionals. Contributes professional nursing perspective in discussions with the interdisciplinary team. Partners with others to effect change and produce optimal outcomes.

Supports colleagues through knowledge sharing to provide safe, quality nursing care. Shares educational findings, experiences, and ideas with peers. Questions clinical practices for the purpose of providing evidence-based care. Participates in activities and strategies to sustain an evidence-based-practice culture.

The CLC RN collaborates with the Minimum Date Set (MDS) coordinator to ensure documentation is accurate and reflective of the Resource Utilization Group (RUG) score. Duties include but are not limited to the following: - Leads and provides nursing care for patients with complex needs using the nursing process and ANA standards. - Uses clinical judgment to guide staff in care planning, decision-making, and outcome evaluation.

- Resolves patient experience concerns, including coordination of care and environment-of-care issues. - Incorporates and advocates for patient, family, and caregiver values in care delivery. - Serves as a resource for addressing and resolving ethical issues. - Leads implementation of HRO principles and I CARE values to improve quality and consistency of care. - Demonstrates effective communication and collaboration to support teamwork and patient safety.

- Participates in and leads interdisciplinary care planning and team conferences. - Promotes staff growth through mentoring, precepting, and leading educational efforts. - Applies evidence-based practice and evaluates care processes to improve outcomes and efficiency. Preferred experience: Long Term and Geriatric experience.
