Job Summary

This position is eligible for the Education Debt Reduction Program (EDRP) Join the VA Black Hills Health Care System in Hot Springs, South Dakota, as a Community Living Center Registered Nurse and make a lasting difference for Veterans.

Provide individualized nursing care, coordinate complex needs, support resident independence, partner with families and interdisciplinary teams, and help strengthen a safe, therapeutic environment focused on quality outcomes and Veteran-centered care.

Key Responsibilities

As a Community Living Center Registered Nurse, you will help Veterans achieve and maintain their highest possible level of health, function, and independence while developing individualized plans of care that reflect their needs, preferences, and goals.

You will collaborate closely with Veterans, families, nursing staff, and interdisciplinary professionals to coordinate care, address complex needs, and promote seamless services. This role also offers opportunities to contribute beyond bedside care by serving as a clinical resource, supporting evidence-based practice, identifying opportunities to improve care delivery, and helping maintain a safe and therapeutic Community Living Center environment.

Duties include, but are not limited to: - Deliver individualized nursing care by assessing resident needs and planning, implementing, and evaluating interventions across the continuum of care. - Develop resident-centered plans of care that incorporate clinical assessments, treatment needs, personal preferences, and goals for maintaining optimal function and independence.

- Coordinate care for Veterans with complex needs, partnering with other services to promote timely, seamless, and effective care delivery. - Administer medications, treatments, and nursing procedures in accordance with established clinical guidelines and standards of practice. - Educate and engage Veterans, families, and caregivers to strengthen participation in care planning, self-management, and informed health care decisions.

- Collaborate with interdisciplinary team members and serve as a nursing resource to support communication, continuity of care, and positive resident outcomes. - Conduct clinical and environmental rounds to evaluate care delivery, assess the therapeutic environment and resident safety, and identify opportunities for program improvement.

- Apply evidence-based practice, nursing research, and current clinical literature to strengthen nursing care, support quality improvement, and advance effective care delivery.

You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.
