Job Summary

The function of the Community Living Center (CLC) Registered Nurse (RN) is to provide, direct, and supervise the delivery of quality nursing care to veterans in the Community Living Center care setting. 9CS RNs work with residents that are challenged with MS, spinal cord injury, Huntington's disease, ALS, cardiac problems, diabetes, and other varied medical issues.

Key Responsibilities

The Community Living Center (CLC) Registered Nurse (RN) is responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. This position is located in the 9CS unit within Geriatric and Extended Care (GEC). Many of the veterans assigned to 9CS have mental health issues, including PTSD, anxiety, dementia, and other behavioral problems.

The unit staff works closely with all members of the interdisciplinary team to provide the Veterans with optimal care. This is the Veterans home and the care delivered is resident centered and honors individual needs and preferences. Veterans remain on the unit for many medical challenges, IV's, and transfusions; avoiding the disruption of transfer to the acute units.

Other duties are, but not limited to: Assuming responsibility for the coordination of care, focusing on patient education, self-management, and customer satisfaction throughout the continuum of care. Administering medications and procedures, per established policies and guidelines. Collaborating with members of the interdisciplinary team. Delivering and improving holistic care through collaborative strategies with others.

Providing peers with informal, constructive feedback for improvement. Participating in activities and strategies to sustain an evidence-based practice culture. Collaborating with the Minimum Data Set (MDS) coordinator to ensure documentation is accurate and reflective of the Resource Utilization Group (RUG) score. Other duties, as assigned.
