Job Summary

The Registered Nurse (RN) Community Living Center (CLC) provides evidence-based care to residents while supervising licensed vocational nurses and nursing assistants. The role supports nursing education, staff orientation, competency development, and quality-improvement initiatives. The RN demonstrates leadership in promoting holistic, collaborative care and contributes to interdisciplinary teamwork to strengthen outcomes within the CLC.

Key Responsibilities

The Registered Nurse (CLC) is responsible and accountable for all elements of the nursing process when delivering or supervising direct patient care. The role coordinates care with an emphasis on patient education, self-management, and customer satisfaction across the continuum of care. The nurse administers medications and procedures per established guidelines and collaborates with the interdisciplinary team to influence care outcomes.

Core duties include active participation in quality-improvement initiatives and customer-service programs. Duties include but not limited to the following: Provides peers with informal constructive feedback for improvement. Fosters a safe and supportive environment conducive to the professional development of healthcare professionals. Contributes professional nursing perspective in discussions with the interdisciplinary team.

Partners with others to effect change and produce optimal outcomes. Supports colleagues through knowledge sharing to provide safe, quality nursing care. Shares educational findings, experiences, and ideas with peers. Questions clinical practices for the purpose of providing evidence-based care. Participates in activities and strategies to sustain an evidence-based-practice culture.

Collaborates with the Minimum Date Set (MDS) coordinator to ensure accuracy and completion of the MDS documentation as required by CLC staff RN. Preferred Experience: 1-2 years of Hospice Experience preferred.
