Delivers fundamental knowledge-based care to assigned clients with terminally illness, end of life, dementia, chronic medical conditions, and other chronic conditions while developing technical skills. Provides a state-of-the-art continuum of services, education, and resources to caregivers of Veterans and Veterans with complex needs.
The Registered Nurse is responsible for: Enrolling and providing telehealth care for Veterans in all Nationally Designated Telehealth Hub (NDTH) programs. NDTH consists of Tele Palliative Care, Tele Post Intensive Care Recovery, Tele Recovery and Resilience Program and Tele Geriatrics. The nurse will screen Veterans for enrollment to NDTH programs using criteria and screening tools.
The nurse will assess using these screening tools and interview questions as to whether the Veteran requires monitoring by the program for their chronic symptoms, palliative care, post intensive care syndrome, and chronic illness. The nurse will contact Veterans daily by video connect and/or phone to conduct interviews, assessments, and schedule appointments.
The nurse will educate Veterans on how to use equipment that includes, but not limited to, pulse oximeter, incentive spirometer and if needed blood pressure machine. The nurse is responsible for their cohort of Veterans providing case management, education, and regular nursing visits to provide care and help Veterans reach their goals. The nurse will work with an Interdisciplinary Team.
The nurse must assess and understand when to report an assessment to the physician, psychologist, pharmacist, social worker, and other disciplines as appropriate. The nurse will help to train new staff and re-educate current staff as needed. The nurse will participate in IDT and family meetings and communicate information about their patients. The nurse will be responsible for the submitting individualized slides for presentation during the IDT meeting.
The program is constantly evolving, and your duties may increase or decrease over time. The nurse is expected to be flexible when needed to provide care to Veterans enrolled in more than one of the NDTH programs. Assess, plans, implements and evaluates care based on age specific components. Assumes responsibility for the coordination of care focused on patient and caregiver education, self-management, customer satisfaction throughout the continuum of care.
Advocates and coordinates for specialized assessments and interventions to improve quality of life, symptom management, family support, and end of life issues. Influences care outcomes by collaborating with team members to include working with outside agencies and resources in the community. Provides and exhibits knowledge in assessment and monitoring, health restoration, wellness promotion, illness prevention, and coping facilitation.
Advocates for holistic care through patient and caregiver education and provider interaction; implements an education plan to meet the patient care needs. RN meets Veterans biological, psychological, social, and environmental needs. May participate in educational sessions to caregivers and Veterans.
Develops and implements information systems to track service activities including visits, referrals, inpatient days, patient demographics, equipment utilization, and patient satisfaction. Develops, implements and evaluates clinical guidelines and protocols to establish appropriate utilization of services to meet the needs of Veterans while supporting the VA mission.

The Associate Degree Nursing (ADN), Bachelor of Science degree in Nursing (BSN), or Master of Science degree in Nursing (MSN) must be fully accredited by the Accreditation Commission for Education in Nursing (ACEN) or the Commission On Collegiate Nursing Education (CCNE).
