The Registered Nurse (RN) -Health Behavior & Veteran Health Education Coordinator is responsible for providing competent, evidence based care to patients with oversight of licensed vocational nurses/nursing assistants as appropriate to the setting. The nurse will assist in directing the provision of nursing education, orientation, competencies and providing quality improvement and enhance outcomes utilization.
Validates understanding of the rationale for care processes and decisions.
Major duties include but are not limited to: Serve as the primary clinical consultant to medical center staff on clinician coaching, motivational interviewing, health behavior change, patient self management, and preventive health. Provide expert guidance and consultation on evidence-based health behavior interventions, coaching methods, and effective patient communication related to health promotion and disease prevention.
Lead, coordinate, and deliver training programs (e.g., Motivational Interviewing, TEACH) for clinical staff to build and sustain skills in clinician health coaching and health behavior change. Contribute to staff development in evidence-based strategies for motivating, coaching, and supporting patients to achieve healthy behavior change.
Plan, implement, and expand the integration of Health and Wellness Coaching, Whole Health, Healthy Living, and Health Promotion/Disease Prevention (HPDP) programs within clinical care. Develop and implement methods to communicate, motivate, and support patients in recognizing health risks, clarifying health goals, and creating tailored Personal Health Plans.
Work collaboratively with Service Lines, Service Chiefs, interdisciplinary teams, and the HPDP Program Manager to coordinate health promotion and disease prevention efforts. Partner with other program leaders to support and strengthen Whole Health and Health Living programming. Assess the efficacy of new and ongoing health behavior interventions and recommend service enhancements as needed.
