Job Summary

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application.

Key Responsibilities

This position is for the Home Based Primary Care (HBPC) Outpatient Staff Registered Nurse (RN) located in Dallas to care for veteran population in the Dallas/Fort Worth area. The HBPC RN provides direct Veteran care and care coordination, as it pertains, to Veterans and their care within the HBPC Program. Responsible for case management, assessment, triage, Veteran education, resource coordination, medication and treatment coordination, and care planning.

The HBPC RN is required to have a state and federal driver's license to operate a government vehicle. Duties include, but are not limited to: Makes home visits to Veterans in the program. Assists in directing the provision of nursing education, orientation, competencies and providing quality improvement and outcomes utilization. Demonstrates leadership in delivering and improving holistic, home-based care through collaborative strategies with others.

Evaluates practice in an ongoing process, based on best evidence. Provides peers with informal constructive feedback for improvement. Fosters a safe and supportive environment conducive to the professional development of healthcare professionals. Contributes professional outpatient nursing perspective in discussions with the interdisciplinary team. Supports colleagues through knowledge sharing to provide safe, quality HBPC nursing.

Shares educational findings, experiences, and ideas with peers. Oversees LVNs/NAs, as appropriate to the setting. Administers medications and procedures per established policies and guidelines. Participates in activities and strategies to sustain an evidence-based-practice culture.

Requirements & Education

EDUCATION ACCREDITATION: The Associate Degree in Nursing (ADN), Bachelor of Science degree in Nursing (BSN), or Master of Science Degree in Nursing (MSN) must be recognized by the appropriate State-accrediting agency and accredited by either of the following accrediting agencies at the time the program was completed by the applicant: -The Accreditation Commission for Education in Nursing (ACEN) OR -The Commission on Collegiate Nursing Education (CCNE) -ACEN or CCNE program or degree accreditation may also be found under the U.S.
