Job Summary

Join the VA Black Hills Health Care System in Pierre, South Dakota, as a Home Based Primary Care Registered Nurse and bring high-quality care directly to Veterans with complex needs in their homes. You'll provide case management, assessment, triage, education, medication and treatment coordination, and individualized care planning while promoting independence and helping Veterans avoid unnecessary urgent and emergency care.

Key Responsibilities

As a Home Based Primary Care Registered Nurse, you will have the opportunity to build meaningful relationships with Veterans while helping them remain safely and independently in their homes. You will serve as an essential member of an interdisciplinary team, coordinating comprehensive care for Veterans with complex health needs and helping align services with their individual goals.

This role offers a rewarding blend of autonomous nursing practice, care coordination, patient education, and collaboration across primary, geriatric, and palliative care while making a direct impact on quality of life and reducing avoidable urgent and emergency care. Duties include, but are not limited to: - Provide skilled, individualized nursing care to Veterans with complex health needs during visits in their homes.

- Manage a caseload of Veterans through comprehensive assessment, care planning, ongoing evaluation, and coordination of services. - Assess and triage Veteran health concerns to identify needs, address changes in condition, and support timely access to appropriate care. - Coordinate medications, treatments, health care services, and community resources to promote continuity of care across settings.

- Educate Veterans and caregivers to strengthen self-management skills, support informed health decisions, and advance individualized goals of care. - Deliver nursing interventions that support health promotion, disease prevention, geriatric care, primary care, and palliative care needs. - Collaborate with interdisciplinary team members to develop and implement coordinated care plans that reflect Veteran preferences, values, and clinical needs.

- Implement strategies that maximize Veterans' health and independence while helping reduce unnecessary urgent care and emergency department visits.

Travel Requirements You may be expected to travel 76% or greater of the time for this position to various care locations within the service area, including Veteran homes as part of Home Health care delivery. You must possess and maintain a current, valid, unrestricted state-issued driver's license. You must provide a safe, clean driving record from the Department of Motor Vehicles (DMV) during onboarding.

You must successfully complete a driver's physical examination to demonstrate medical ability to safely operate a government-owned or leased vehicle, as required under 5 CFR 930 and HR Job Aid 05-OB-010.002.
