This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. The HUDVASH Case Manager works in an intensive case management program for chronically homeless Veterans aimed at providing bio-psychosocial services to Veterans while residing in stable housing through HUD's Section 8 voucher system. The program funding provides rental assistance under a supportive housing program for homeless and combines rental assistance for homeless Veterans with case management/clinical services.
The HUDVASH Case Manager (CM) Registered Nurse (RN) executes position responsibilities that demonstrate leadership, experience, and creative approaches in providing complex patient management. Provides patient management, care coordination, and discharge/disposition planning for inpatient and outpatient settings. Assists Veterans, family members, and caregivers with receiving the most appropriate options and services to meet their complex health care needs.
Coordinates care with multiple providers across all levels and sites of care. Addresses psychosocial, as well as nursing and medical needs of patients and their families/caregivers, through participation in interdisciplinary patient care management practice. Evaluates care and outcomes to ensure timely and appropriate provision of services. Collaborates in the development, implementation, evaluation, and revision of policies, procedures, and/or guidelines.
Evaluates HUDVASH CM outcomes of evidence-based decisions and practice changes for individuals, groups, and populations. Responsible for the documented CM outcomes at the program or service level. Program or service level outcomes must be broad and complex and can be demonstrated at any organizational level within a facility. Works closely with Program Manager and the HUD/VASH Treatment Team to provide case management to Veterans in the HUD/VASH program.
Performs site visits of HUD/VASH apartments/homes to ensure that Veterans reside in safe environments in compliance with local housing codes. Verifies the Veteran's status, eligibility for VA medical care, and eligibility of program participation. Ensures that chronically homeless Veterans are considered as primary participants for program acceptance. Collects and submits HUD/VASH Program participant data as outlined by NEPEC evaluation procedures.
Coordinates care including serving as an advocate on behalf of the Veterans to ensure comprehensive service delivery and linking and referring Veterans to the VA Medical Center, VA Regional Offices and/or community agencies. Works with Program Manager and HUD/VASH Treatment Team to Meet/Exceed HUD/VASH Performance measures that includes issuing vouchers to chronically homeless veterans and maintains 88% of Veterans in caseload housed.
Conducts nursing assessments to determine status and needs of medical as well as psychosocial conditions of Veterans and family members. Participates in the treatment planning process with other disciplines to include discharge planning.

At least 70% of time is spent in the community; 75% or less maybe expected to travel.
