This position is located at the Audie L. Murphy Memorial Veterans' Hospital in San Antonio, TX. The Case Management (CM) Registered Nurse (RN) provides patient management, care coordination, and discharge/disposition planning for inpatient/outpatient settings. The CM RN is in a growth and development practice on assisting Veterans, family members, and caregivers with receiving the most appropriate options and services to meet their complex health care needs.
Nurse I, Level 1 Provides fundamental inpatient nursing care with close supervision using the nursing process. Assesses patient needs, and evaluates responses to care. The CM RN addresses psychosocial, as well as nursing and medical needs of patients and their families/caregivers, through participation in interdisciplinary patient care management practice. The CM RN also evaluates care and outcomes to ensure timely and appropriate provision of services.
The nurse brings principles and theory with limited application of critical thinking skills. Nurse I, Level 2 Provides patient management, care coordination, and discharge/disposition planning for inpatient and outpatient settings. Coordinates care with multiple providers across all levels and sites of care.
The CM RN addresses psychosocial, as well as nursing and medical needs of patients and their families/caregivers, through participation in interdisciplinary patient care management practice. The CM RN has fundamental experience with the situations in which they are ultimately expected to perform, integrating theoretical concepts, basic critical thinking skills, and technically skilled practice.
Nurse I, Level 3 Provides patient management, care coordination, and discharge/disposition planning for inpatient and outpatient settings. The CM RN assists Veterans, family members, and caregivers with receiving the most appropriate options and services to meet their complex health care needs.
This includes, but is not limited to, acute, chronic, multiple, complex, catastrophic, or life-threatening illnesses; combat stress, residuals of traumatic brain injury; community adjustment; addictions and other health problems. Coordinates care with multiple providers across all levels and sites of care. Functions as an emerging leader or charge nurse by directing staff and coordinating care. Validates understanding of the rationale for care processes and decisions.
Questions clinical practices for the purpose of providing evidence-based care. Educates colleagues and students, mentors staff, and supports evidence-based performance improvement. Nurse II The CM RN also evaluates care and outcomes to ensure timely and appropriate provision of services. Demonstrates leadership in delivering and improving holistic care through collaborative strategies with others. Evaluates practice in an ongoing process, based on best evidence.
Provides peers with informal constructive feedback for improvement. Fosters a safe and supportive environment conducive to the professional development of healthcare professionals. Contributes professional nursing perspective in discussions with the interdisciplinary team. Supports colleagues through CM knowledge sharing to provide safe, quality nursing care. Shares educational findings, experiences, and ideas with peers.
Questions clinical practices for the purpose of providing evidence-based care. Participates in activities and strategies to sustain evidence-based-practice. Nurse III The Nurse III Case Management (CM) Registered Nurse (RN) executes position responsibilities that demonstrate leadership, experience, and creative approaches in providing complex patient management.
Collaborates in the development, implementation, evaluation, and revision of policies, procedures, and/or guidelines. Demonstrates performance and leadership that is broad enough to improve the care for a group of patients. The Associate Degree Nursing (ADN), Bachelor of Science degree in Nursing (BSN), or Master of Science degree in Nursing (MSN) must be fully accredited by the Accreditation Commission for Education in Nursing (ACEN) or the Commission On Collegiate Nursing Education (CCNE).
