The Intensive Community Mental Health Program Nurse is a member of the Behavioral Health Care Service Line whose role is to collaboratively manage, coordinate and provide health care to a defined group of high risk patients with diagnosed severe mental illnesses and substance use disorders. Care is provided to patients with the goal of achieving quality patient care by improving clinical outcomes, increasing continuity of care and minimizing health care costs.
The ICMHR Nurse is accountable to the ICMHR Nurse Manager for providing the highest quality of nursing care possible for patients. This responsibility includes the utilization of general clinical nursing skills and knowledge in the area of care coordination of patients and education of patients/significant others as indicated. Major Duties include, but are not limited to: Assesses and triages medical needs of all ICMHR patients as indicated. Advocates for patient needs on a case by case basis including accompanying patients to medical appointments. Provides care in the Veterans community through assertive outreach and transportation of patients as needed. Demonstrates understanding of infection control precautions. Assesses, plans, implements, evaluates, and documents and to promotes health and achievement of treatment goals and includes collaboration with the multidisciplinary team. Develops and maintains therapeutic relationships with patients, families, and the interdisciplinary team. Promotes the medical center mission, vision, and values; complies the Equal Employment Opportunity programs, partnerships, customer service standards and Veteran Affairs policies and procedures. Provides community-based clinical case management for patients who have severe and persistent mental illness with a significant history or frequent psychiatric hospitalizations. Administers prescribes therapies using appropriate judgement; evaluation of therapies is persistent and timely. Demonstrates and/or verbalizes correct responses to emergencies, manages complex patient situation and initiates effective action emergencies. Participates/leads unit-based shared governance, performance improvement, and safety initiatives.
