This Registered Nurse Manager position is in the Nursing-GEC service of Home-based Primary Care (HBPC) services at the VA Connecticut Healthcare System, located at the West Haven, CT campus. This position is full-time at 40 hours per week.
The Home-Based Primary Care (HBPC) Registered Nurse Manager is responsible for providing competent, evidence-based care to residents with oversight of registered nurses as appropriate to the setting. The Registered Nurse Manager will assist in directing the provision of nursing education, orientation, competencies and providing quality improvement and enhance outcomes utilization.
Duties include but are not limited to: Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Administers medications and procedures per established policies and guidelines. Influences care outcomes by collaborating with members of the interdisciplinary team.
Core elements of performance include knowledge and active participation in the unit/program level quality improvement process and initiatives as well as customer service programs. Responsible and accountable for individual nursing practice and determines the appropriate delegation of tasks consistent with the nurse's obligation to provide optimal care. The focus is on professional organizations to enhance leadership and career growth.
Ability to formulate a plan of care based on nursing assessments. Seeks guidance in how to improve care provided to residents and families. Brings principles and theory with limited application of critical thinking skills. Maintains knowledge and ensures compliance with all regulatory and accreditation standards, Directives, policies, and standard operating procedures (SOP).
