The PACT Care Manager Nurse uses the nursing process and evidence-based practice to collaborate with the team (Patient, Primary Care Provider, PACT RN Care Manager, Health Technician, Pharmacists, and Clerical Associate) and expanded team (family member or caregiver, consulting providers, internal and community-based services involved in the care of the patient) in developing the patient-driven care plan for prevention and management of diseases.
This position is located at the Charlotte Hall VA Clinic in Charlotte Hall, MD. This outpatient clinic provides primary care and specialty health services, including mental health care, nutrition counseling, social work, telehealth services, and more. Major Duties/Functions include but not limited to: Responsible for the collection, analyzing of data and implementing evidence based changes to practice based on data results.
Works in partnership with an assigned panel of patients and team members for effective disease and chronic care management. Responsible for collaborating with services internal and external to the VA to facilitate care transition in order to effectively meet the patients' needs. Provides evidenced-based patient and family health education with a focus on self-management, prevention, and wellness based on the patient's goals.
Demonstrates leadership by service as an advocate for patients, and a team leader with colleagues. Initiates follow-up as recommended while administering the prescribed therapies using appropriate judgment; evaluation of therapies is pertinent and timely. Provides care in a non-judgmental, non-discriminatory manner that is sensitive to the adult or elderly patient and family's lifestyle, preserving their autonomy, dignity and rights.
Focuses on problems, initiates referrals to other disciplines and/or services as appropriate. Accurately documents in the medical record patient findings, assessments, and care provided. Demonstrates and/or verbalizes correct responses to emergencies as well as initiates effective action in emergencies. Demonstrates understanding of infection control precautions appropriate for the patient population.
