The Registered Nurse (RN) is responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Assesses, plans, implements, and evaluates care based on age-specific components. The RN assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. The RN must work well within a team environment.
The Intermittent Registered Nurse Patient Aligned Care Team (PACT) is a self-directed professional nurse who assumes a leadership role in both the direct and indirect management of a group of patients by coordinating care with physicians, nurse practitioners, medical residents/students, other health professionals, and hospital systems. This role spans all sites in managed care nursing and reports directly to the site Nurse Manager of Managed Care.
The RN is responsible for coordinating the care of patients assigned to the PACT. Within the PACT, the RN provides intensive management for at-risk patient subgroups who may be admitted as inpatients and subsequently discharged to the outpatient setting, or who remain in the outpatient setting. The RN demonstrates leadership, experience, and creative approaches to managing complex care through their position responsibilities.
They coordinate patient care across and between healthcare systems, including the private sector, continuing to exhibit leadership, experience, and innovative care management. The RN supports patient self-management, prevention, and wellness based on the patient's goals, utilizing motivational interviewing skills to help patients establish and achieve these goals.
