Job Summary

The Davenport VA Clinic is recruiting for a Patient Aligned Care Team (PACT) Outpatient Staff Registered Nurse (RN). PACT RN provides direct Veteran care and care coordination, as it pertains, to Veterans and their care within the Primary Care Department.

Key Responsibilities

The VA Midwest Health Care Network advocates for a Whole Health System of care in each of the Medical Centers. This is an approach to healthcare that empowers and equips people to take charge of their health and well-being and live their lives to the fullest. As an employee operating in a Whole Health System of care, you will operate in a model with three core elements, seeking to create a personalized health plan for each Veteran.

This is done in the context of healing relationships and healing environments and a connection back to the Veteran's community. This aligns with the Veterans Health Administration (VHA) Mission Statement to Honor America's Veterans by providing exceptional health care that improves their health and well-being. Major duties include, but are not limited to: Provides comprehensive nursing care and care coordination within the Primary Care PACT.

Serves as the primary RN Care Manager for a designated patient panel, ensuring continuity, coordination, and quality of care. Delivers proactive, patient-centered care using nursing knowledge, evidence-based guidelines, and standardized protocols. Focuses on patient education, self-management support, health promotion, chronic disease management, and overall wellness.

Collaborates with PACT team members, extended care teams, and community/consulting services to develop holistic, Veteran-driven care plans. Oversees transitions of care across modalities including face-to-face visits, virtual care, secure messaging, and telephone-based services. Develops and implements Veteran-centered care plans and chronic disease management interventions.

Uses population health tools to identify and manage high-risk or chronically ill patients, collaborating with interdisciplinary team members to optimize outcomes. Applies structured appointment grids and engages in panel management to support self-management and reduce avoidable complications. Facilitates care transitions and ensures timely access to needed health care resources.

Advocates for patients and engages in patient/family education to improve outcomes and satisfaction. Demonstrates leadership in professional practice, evidence-based practice, quality improvement, and resource utilization.
