The Registered Nurse (RN) Patient Care Coordinator (PCC) is responsible for overseeing and coordinating patient care activities to ensure safe, effective, and high-quality care delivery. This role serves as a clinical resource, liaison, & patient advocate, facilitating communication among patients, families, interdisciplinary team members, and healthcare providers.
The RN PCC supports care planning, monitors patient progress, and helps ensure continuity of care across services and settings.
Major Duties Include: Establishes a therapeutic relationship, allowing the patient to achieve optimal function through assessment and treatment of their health enhancing the patient experience, satisfaction, and/or safety. Treats all patients, families and other caregivers with dignity and respect incorporating knowledge, values, and beliefs into nursing care delivery without judgement or discrimination.
Leads the provision of nursing care to patients with complex care needs utilizing the nursing process in alignment with the ANA Standards of Nursing Practice. Assesses the immediate practice setting to identify efficient, cost-effective care delivery. Leads the integration of evidence-based practice/research with clinical expertise to improve quality of care and resource utilization with improved outcomes for the immediate practice setting.
Leads professional practice using the nursing process (assessment, diagnosis, outcome identification, planning, implementation, and evaluation) to improve evidence-based and/or data-driven outcomes for an identified patient population and/or specialized areas within immediate practice setting. Provides and directs others in Veteran/patient driven care by optimizing the patient experience, satisfaction, and/or safety for a team of patients.
