The Registered Nurse (Restorative Coordinator)/Community Living Center (CLC) is responsible for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Administers medications and procedures per established policies and guidelines.
The Restorative Coordinator provides competent, evidence-based care to assigned residents and oversight of licensed vocational nurses/nursing assistants, as appropriate to the setting. Other responsibilities and duties include but are not limited to the following: Assists in directing the provision of nursing education, orientation, competencies and providing quality improvement and outcomes utilization consultation.
Demonstrate leadership in delivering and improving holistic care through collaborative strategies with others. Completes the restorative nursing sections of the Minimum Data Set (MDS), specifically the observation period, quarterly reassessments, and significant changes in care. Monitors assignments to ensure they are appropriate with the level of education and training demonstrated of restorative personnel.
Evaluates patients in the CLC upon admission to Restorative Care Program and monthly thereafter to determine the potential for the program. Responsible for screening Veterans and conducting initial assessment prior to admission to the Restorative Care Program. Outline and implement measurable functional goals for Veterans to reach their highest level of function and maximize their independence.
Collaborates with educators, Occupational Therapy (OT), Physical Therapy (PT) in recruiting and admitting patients to the Restorative Care Program and develops goals to maximize the patient's independence and functioning. Evaluates patient progress toward their goals monthly and institutes changes in therapy, goals, and activity when necessary. Provides developmental opportunities for program staff to acquire clinical restorative nursing skills and expertise.
