Job Summary

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application.

Key Responsibilities

THIS IS NOT A REMOTE POSITION! Registered Nurse (RN) - Referral Coordination Team (RCT) is responsible for executing a streamlined approach to receiving, triaging, and directing timely, coordinated care.

The person in this role possesses intimate knowledge of the internal referral care process, clinical review criteria, utilization management standards, clinical documentation requirements, community care regulations, and Veterans Health Administration (VHA) standards and guidelines.

The RN - RCT uses nursing knowledge and systems theory to assess, organize, facilitate, and guide Veterans through their full range of care options including internal/direct care in Veterans Affairs (VA) and care in the community. The RN - RCT is also responsible for the following duties: Serving as a liaison to both internal and community providers and being responsible for managing the care of the Veteran throughout the consult process.

Supporting the medical center's mission to improve timely access to care, empowering Veterans to make more informed care decisions and ensuring eligible Veterans who want to receive care in the community are referred and scheduled into the community. Triaging incoming referrals, discussing available care options, providing scheduling guidance, and being a resource within the service.

Guiding Veterans through their full range of care options, including internal/direct care in VA and care in the community, by utilizing a national standardized triage tool. Serving as the liaison between ordering or referring VA providers and specialty care services by clinically triaging consults or referrals to confirm the appropriate care is requested and all necessary pre-work and tests are completed.

Complying with VA mandated use of health information solutions that serve to store, exchange, and track referral care information. Working collaboratively with the Office of Integrated Veteran Care to triage Request for Services (RFS) to promote in-house service access. Other duties, as assigned.

The Accreditation Commission for Education in Nursing (ACEN), located at 3343 Peachtree Road NE, Suite 850, Atlanta, GA 30326.

The ACEN accredits all levels of nursing programs, including clinical doctorate, masters/post master's certificate, baccalaureate, associate and diploma nursing education programs. (Note: The National League for Nursing Accrediting Commission/NLNAC changed its name to ACEN in May 2013.

ACEN's website may be utilized to verify accredited programs using automated inquiry processes; however, for verification of any nursing program's accreditation prior to 1998, an automated inquiry cannot be used and ACEN must be contacted directly via their email contact information on the ACEN website to verify.) The Commission on Collegiate Nursing Education (CCNE), located at 655 K Street, NW, Suite 7, Washington, DC 20001.

The CCNE is an accrediting arm of the American Association of Colleges of Nursing (AACN) and accredits nursing education programs at the bachelor and master's level.
