Job Summary

The Registered Nurse will assess, plan, implement and evaluate care based on age-specific components while assuming the responsibility for the coordination of care focused on patient education, self management, and customer satisfaction throughout the continuum of care.

Focus on individual growth and development in practice with increased levels of self-direction. Provide patient care, care coordination, health promotion, health education and coaching on wellness, disease prevention, and chronic care management. Ability to formulate a plan of care based on nursing assessments and Veteran goals of care. Seek guidance on how to improve care provided to patients and families. Delegate elements of care to the appropriate individual based upon assessed needs, condition, and safety of the patient. Bring principles and theory with limited application of critical thinking skills. Perform a wide range of basic skilled nursing care to a variety of patients as assigned in a medical or surgical ambulatory care setting. Ability to communicate, listen, report and gather patient data. Specialties represented in this setting include but are not limited to: Cardiology, Dermatology, ENT, General Surgery, Heart Failure/Transplant, Allergy, Hematology/Oncology, Neurology, Pulmonology, Infectious Disease, Neurosurgery, Orthopedics, Orthopedic Hand, Orthopedic Spine, Podiatry, Rheumatology, Thoracic, Urology and Vascular Surgery Service. Use the nursing process to assess, plan, implement, and evaluate patients. Demonstrate leadership in the education of the patient, family/significant other. Document coordination of care provided in CPRS and other appropriate resources in a timely manner. Function as an integral part of the service team, working in collaboration with the nurses, physicians/providers, social workers and other health care professionals to enhance the overall care of the patient. Identify potential emergency/crisis situations and provide appropriate interventions. Participate in scheduling patients for outpatient clinic appointments, procedures, surgery and necessary testing as applicable and as directed by service protocols. Assist with completing clinic intake, maintaining clinic flow for various clinics within Specialty Care and administering treatments for assigned groups of patients within scope of practice and according to facility policies and procedures. Set up for ad assist with outpatient surgical procedures. Competent and compliant with sterile technique. Assist in team research of quality improvement projects and updating service related policies and protocols. Seek out and participate in committees and task forces as needed/appropriate. Participate in program and clinic specific activities and meetings. Rotate Charge Nurse responsibilities as directed by Nurse Manager. Other duties as assigned.
