The Outpatient Staff Registered Nurse (Specialty Care) provides and coordinates specialty outpatient nursing care using the nursing process. Leads evidence-based practice, patient education, and care coordination. Collaborates with interdisciplinary teams to improve outcomes and supports professional development while ensuring safe, high-quality care.
Major Duties Direct Patient Care: Assess, plan, implement, and evaluate care based on age-specific components. Administer medications and procedures per established policies and guidelines. Care Coordination: Assume responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Leadership & Collaboration: Demonstrate leadership and experience in managing complex specialty care.
Collaborate with interdisciplinary teams to influence care outcomes and improve specialty care for outpatients. Health Promotion & Education: Provide health promotion, education, and coaching on wellness, disease prevention, and chronic care management. Policy & Performance Improvement: Participate in the development, implementation, evaluation, and revision of policies, procedures, and guidelines.
Lead and organize delivery of care to ensure continuity and peer accountability. Mentoring & Professional Development: Provide constructive feedback to peers, support colleagues through knowledge sharing, and foster a safe and supportive environment for professional development. Evidence-Based Practice: Lead the identification and integration of evidence-based practice and research to improve quality of care and resource utilization.
Customer Service: Consistently communicate and treat customers (patients, visitors, volunteers, staff) in a courteous, tactful, and respectful manner. Handle conflict constructively. Age-Specific Care: Provide care appropriate to the age, developmental, and lifestyle needs of patients, including young adulthood, middle adulthood, and late maturity.
Physical Requirements: Perform duties requiring physical activity such as walking, standing, stooping, bending, sitting, reaching, pushing, and pulling. Moderate lifting/carrying up to 35 pounds, following Safe Patient Handling guidelines.
Care Coordination Lead (when appropriate): May serve as Care Coordination and Integrated Case Management (CCICM) Lead Coordinator for Veterans requiring moderate to complex care coordination, ensuring seamless and comprehensive care across VA service lines. Preferred Experience: Candidates with experience in circulating and scrubbing, supporting surgical workflows, and maintaining sterile technique will be considered highly qualified.
