Direct patient care - Provides competent, evidence-based nursing care to assigned SCI patients using the full nursing process (assessment, diagnosis, outcome identification, planning, implementation, evaluation). Medication and treatment administration - Administers medications and procedures per established policies and clinical guidelines. Clinical oversight - Supervises licensed vocational nurses and nursing assistants as appropriate to the setting.
Care coordination - Coordinates patient care across the continuum, focusing on education, self-management, and satisfaction; may serve as CCICM Lead Coordinator for Veterans with complex care needs. Patient/family education - Leads patient and family education efforts to support self-management and informed participation in care. Clinical decision-making - Applies clinical knowledge and judgment to guide planning and evaluate patient outcomes.
Patient advocacy - Advocates for patients, families, and caregivers, incorporating their values and preferences into care delivery without judgment or discrimination. Evidence-based practice - Applies current evidence and best practices at the bedside to improve patient outcomes. Age-appropriate care - Assesses and delivers care suited to each patient's age, developmental stage, and lifestyle needs.
Documentation and data security - Protects sensitive patient information (PHI/PII) in accordance with the Privacy Act and VA policy while managing patient records and access.
