Job Summary

The Transcranial Magnetic Stimulation Registered Nurse executes their position duties by providing patient-centered and recovery-oriented care and maintaining self-awareness while caring for patients living with Mental Health conditions.

https://www.youtube.com/watch?v=NpaTtyybXFQ Major Duties: Duties and responsibilities include, but are not limited to, the following: Ensure completion of the initial screening, which consists of verifying the Veteran's identity and assess for changes in medications, side effects, medical conditions, psychiatric conditions, and suicidal or homicidal thoughts. Provide daily screenings of neuromodulation patients.

This screening includes: inquiries regarding changes in medication, sleep status, mood or anxiety related to neuromodulation therapy; assess for use of substances and knowledge of medication/substance use relevant to neuromodulation therapy; and evaluate the veteran for any worn metal that needs removal. Provide pre-treatment education for all interested Veterans and potential new patients.

After the rTMS Psychiatrist/Psychiatric Nurse Practitioner determines a patient is clinically appropriate, has no contraindications, would benefit from rTMS therapy, and has written the order for rTMS therapy in the Computerized Patient Record System, the rTMS Clinical Nurse Treatment Operator will provide procedure instructions to the patient.

Identify patients, verify that proper informed consent has been obtained, obtain patient consent for treatment, and document consent in the patient record. Provide support to the rTMS Psychiatrist/Psychiatric Nurse Practitioner or their designee during motor threshold (MT) determination. This requires familiarity with MT procedures, including careful clinical observation and assistance with identification of rTMS-induced movement.

It also requires ability to consider changes in clinical status that might impact MT and alerting the rTMS Psychiatrist/Psychiatric Nurse Practitioner. Gather medical history and other required pertinent information prior to and during rTMS therapy; administer mental health rating scales and other assessments as needed for clinic patients. Conduct, coordinate, or order various medical tests and labs according to protocol.

Administer rTMS treatments under the supervision of the rTMS Psychiatrist/Psychiatric Nurse Practitioner; provide supervised daily operations of rTMS Clinic devices. Identify any significant changes prior to and during treatments. Ensure the mental health safety of patients throughout the course of their rTMS therapy until such time the patient's care returns solely to the care of their Mental Health Attending Psychiatrist.

File electronic medical record treatment notes daily in CPRS, document the procedures performed under the appropriate note title following established forms filled with relevant data from the day's treatment. Demonstrate abilities to deliver basic emergency care in the rTMS Clinic, including use of the emergency button on the rTMS therapy device and implementation of facility emergency procedures.

Serve as a primary contact for patients and family members throughout their evaluations and courses of treatment. Provide operational support for the rTMS Clinic, including monitoring clinic supplies, ensure quarterly reviews of available supplies, and coordinate with inventory management, and respond to voicemails and e-mails in a timely manner. Contribute to approved neuropsychology and neuromodulation research protocols and program evaluation projects.

Maintain annual competencies required by the Mental Health Service in the rTMS RN education and training folder and the vendor's rTMS System training records. May include driving mobile unit to various locations to support and provide rTMS treatment throughout VAMT.
