Job Summary

This RN, Clinical Assessor position is for the Caregiver Support Program which is under the Social Service Department at the VA White River Junction Healthcare System. This position is full-time at 40 hours per week. NOTE: The 2-page Resume requirement does not apply to this position. For more information, refer to Required Documents below.

Key Responsibilities

This RN, Clinical Assessor position is for the Caregiver Support Program which is under the Social Service Department at the VA White River Junction Healthcare System. This position is full-time at 40 hours per week. Included but are not limited to: Clinical Assessor executes position responsibilities that demonstrate beginning leadership skills and participates in specific PCAFC experiences.

Demonstrate creative approaches to the management of aspects of complex patient care. Provides services of persons in the promotion and maintenance of health and prevention of illness and injury based upon the nursing process which includes systematic data gathering, assessment, appropriate nursing judgment, and evaluation of human responses to actual or potential health problems through such services as case finding, health teaching, and health counseling.

Possesses thorough knowledge of PCAFC, PGCSS, clinical documentation requirements, current standards of care, and all regulatory compliance requirements and guidelines. Accountable for utilizing critical thinking skills and sound judgment to ensure Veterans and caregivers are provided with thorough, comprehensive care options within the scope of the CSP.

The CSP Clinical Assessor conducts home visits to caregivers and Veterans approved in the program and provides assessments at VA facilities, via phone and telehealth. Provides referrals and/or consults to VA and community supports and services. Demonstrates knowledge and ability to apply principles of the CSP practice.

Applies the nursing process and care coordination principles within a collaborative, interdisciplinary practice setting, which will then enable effective application of CSP practices and principles. Core functions include thorough assessment of Veteran and Caregiver needs to determine the most appropriate components of program participation.

Uses knowledge of the CSP to matrix Veteran level of functioning with the appropriate CSP services (i.e., PGCSS; PCAFC; and/or Veteran entitlements and benefits) to ensure patient safety, recovery, and program stewardship. Evaluates, systematically, current practice and formulates outcomes for groups of patients and/or organizational processes within area of expertise.

Coordinates within the CSP to develop and implement strategies for performance improvement and enhancement of the program Work Schedule: Monday-Friday 8:00am-4:30pm Compressed/Flexible: May be available at supervisor's discretion Telework: May be available on an Ad-Hoc basis at supervisor's discretion Virtual: This is not a virtual position.

Requirements & Education

The Associate Degree Nursing (ADN), Bachelor of Science degree in Nursing (BSN), or Master of Science degree in Nursing (MSN) must be fully accredited by the Accreditation Commission for Education in Nursing (ACEN) or the Commission On Collegiate Nursing Education (CCNE).
