The Community Living Center (CLC) Registered Nurse serves as a clinical leader responsible and accountable for the delivery and oversight of complex resident care in a long-term care setting. The incumbent applies advanced clinical expertise, leadership, and evidence-based practice to improve patient outcomes, enhance care delivery processes, and support organizational performance goals.
The RN collaborates across disciplines to coordinate care, promote patient-centered outcomes, and lead quality improvement initiatives that positively impact care beyond the immediate practice setting. Major Duties Provides assessment, planning, implementation, coordination, and evaluation of resident care. Collaborates with interdisciplinary team members to coordinate care and improve outcomes. Uses evidence-based practice to support quality care and patient safety.
Advocates for Veterans, families, and caregivers while providing patient-centered care. Collaborates with the Minimum Data Set (MDS) Coordinator to ensure accurate documentation and Resource Utilization Group (RUG) scoring.
