Job Summary

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application.

Key Responsibilities

The Senior Licensed Professional Mental Health Counselor - ICMHR Case Manager delivers intensive, community-based case management to Veterans with serious mental illness, providing individualized recovery services, therapy, assessments, and crisis intervention. Working independently, the Case Manager collaborates with VA and community partners to promote health, independence, and community integration for Veterans with complex needs.

Major duties and responsibilities of the position include but are not limited to: Provide individual, group, and family counseling to patients and families with complex emotional, medical, and social issues. Independently address and resolve complex patient and family problems. Assess psychosocial functioning and needs; formulate and implement treatment plans identifying strengths, weaknesses, coping skills, and assistance required.

Apply various treatment modalities to achieve goals with individuals, families, and groups. Utilize supportive, problem-solving, and crisis intervention techniques; independently manage mental health crises and support others. Conduct screening and assessment of Veteran referrals; determine clinical need and present findings to the ICMHR treatment team.

Deliver intensive case management in homes and the community, focusing on rehabilitation, crisis management, skill building, and increased independence. Provide specialized treatment and advanced case management for Veterans with serious mental illness and co-occurring psychiatric, medical, and psychosocial conditions. Partner and collaborate with VA and community agencies to ensure continuity of care; act as a resource and liaison.

Address Veteran needs or concerns collaboratively. Operate government vehicles for home and community visits and to transport Veterans related to care plans. Work Schedule: Monday - Friday, 8 AM - 4:30 PM Compressed/Flexible: May be available at supervisory discretion. Telework: Ad-hoc telework may be available at supervisory discretion.
