James A. Haley Veterans' Hospital is looking for a Senior Social Worker Program Coordinator who will be assigned to the HUD-VASH program. This position works with an interdisciplinary team and provides screening and consultative services to the team and other staff regarding the psychosocial and homeless needs of veterans and the impact that homelessness has on their well- being.
The incumbent manages and controls the use of data related to these programs regarding day-to-day functioning.
The selected Social Worker program coordinator oversees the daily operation of the program, develop policies and procedures for program operation, and prepare reports and statistics for the facility, VISN, and national use. They may be responsible for the program's budget, developing and monitoring staff compliance with practice, standards and guidelines on documentation, workload, data entry, ethical practice and service delivery.
Additional duties include, but are not limited to; Program coordinators at the GS-12 grade level are administratively responsible for a clinical program providing treatment to Veterans in a major specialty area, such as but not limited to: Spinal Cord Injury, Homeless Continuum Veteran Program, Hospice and Palliative Care Program, Suicide Prevention Program, Veterans Justice Outreach, Caregiver Support Program and Community Nursing Home Program.
The program coordinator provides analysis and evaluation of clinical program data and computerized programs to identify system-wide trends and needs to enhance the quality of service. They may be responsible for, or contribute to, the program's resource and fiscal management, monitoring control points developing the annual budget, operating within that budget, and accounting for appropriated funds.
The program coordinator is administratively responsible for the clinical programming and prepares reports and statistics for facility, VISN, and national use. They provide leadership, direction, orientation, coaching, in-service training, staff development, and continuing education programs for assigned staff. They initiate and conduct a variety of program or service audits and complete designated clinical practice audits and reports, including productivity assessments.
They oversee program operations and evaluations, identifying areas for improvement, gathering relevant data, assessing the data, developing and implementing ideas for improvement and evaluating efficacy of improvement efforts. Incumbent meets the needs of customers while supporting VA MIssIons. Consistently communicates and treats customers (veterans, their representatives, visitors, and all VA staff) in a courteous, tactful, and respectful manner.
Incumbent provides the customer with consistent information according to established policies and procedures. Handles conflict and problems in dealing with the consumer constructively and appropriately. Adhere to Service Line Agreements, (SLA) Standard Operating Procedures (SOP) for documentation.
Adhere to approved templates and required standards for documentation as outlined by accrediting bodies such as Joint Commission, GARF, and/or revenue sources providing funding to VHA. Provide documentation sufficient to meet the needs of various interdisciplinary teams engaged in the discharge planning process both within the facility and for potential placement and/or aftercare programs/facilities.
Adhere to SLA, SOP, and directives for clinical contact with patients, families, and collateral providers over the course of treatment, which includes pre-admission, program participation, and post discharge follow up. Provide individualize care coordination and discharge planning highlighting patients strengths, needs, abilities, and preferences within patient interaction, Interdisciplinary Team (IDT) meetings, along with family and collateral contacts.
Advise supervisor and IDT on identified treatment goals, priorities, and disposition options and progress. The GS-12 Social Worker Coordinator performs duties independently. The incumbent reports to the assigned Supervisor in social work service and to the Assistant Chief / Chief of Social Work. Incumbents may receive day to day oversight from an Assigned POC on their matrix/assigned teams, and this individual may be from another discipline.

Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may NOT be substituted for the master's degree in social work.
