This position is a Social Worker with duty station located at the Sisseton Service Unit, Sisseton, South Dakota. This position reports to the Behavioral Health Supervisor or Designee. A REAL ID will be required beginning May 7, 2025, in accordance with 6 C.F.R. 37.5 (2021).
Assess and evaluate patients using a patient centered medical home approach using therapeutic techniques to diagnose and treat mental health diagnosis. Develops interventions based on above need assessment and exercises mature professional judgment in consultation with the supervisory social worker in providing therapeutic modalities that are culturally appropriate. Participates and when appropriate takes a leadership role in assessing community human service needs. Works with community members and human resources -providers to develop approaches to addressing the needs of patients. Assists in monitoring and reviewing services for quality and appropriateness of care. Assists in developing as needed appropriate services to address patient and family psychosocial needs. Prepares reports as requested on quality improvement activities.
