The Social Worker practices in the outpatient primary care program, Patient Aligned Care Team (PACT). The PACT social worker operates within the outpatient primary care clinic at the Medical Center.
Key Responsibilities
Major duties include, but are not limited to: Provides outpatient support and assistance to Veteran's in need of a variety of social services. Collaborates with the multidisciplinary team for effective service provision. Provides education and implementation of Advance Directive for Heatlhcare. Community nursing home placements which includes respite care placements. Connects Veterans to food resources and meal delivery programs.
Provides homeless resources such as shelters, meals, HCHV. Psychosocial assessments for Veteran's who are eligible for the VA programs. Provides mental health assessment and suicide prevention of Veterans. Performs other related duties as assigned. Work Schedule: Monday through Friday, 7:00AM to 3:30PM. Telework: Not Available.
