Job Summary

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment.

Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

Key Responsibilities

The Social Worker - HUD-VASH (Housing and Urban Development-VA Supportive Housing) Case Manager for the Homeless Service Program at the Bath Campus provides services to patients and their families to address psychosocial diagnosis and needs.

The primary function of the HUD-VASH Case manager is to assess, treat, and provide on-going case management services to Veterans, the veteran's family members/significant others while collaborating with the interdisciplinary treatment team to ensure the veteran is ready for discharge and re-integration into the community and prevent future homelessness.

Responsibilities include, but are not limited to: Provides case management, clinical treatment, advocacy, and coordination of linage with appropriate VA and community service providers/agencies as needed by the veteran, on an independent practice level.

Provides education to residents, interns, and other in-training staff on the role of social work and discharge options available in the community Completes psychosocial assessments as required in the social work service line documentation policy using interviews with the veteran, family members/significant others and/or other service providers whenever possible and with the veteran's permission.

Develops an interdisciplinary treatment plan, in collaboration with the veteran, veteran's family members/significant others, and other treatment team members. Completes on-going reassessments and updates of the treatment plans due to the changing needs of the veteran and accomplishments of preliminary goals and/or development of new goals, and improvement of mental health status.

Provides case management services to veterans utilizing HUD/VASH vouchers, to include individual, group, family supportive counseling and psychosocial education. Documents the veteran's progress as well as any services provided via electronic process notes. Provides referrals to service providers throughout the course of treatment, both within and outside of the VA, based on need and eligibility.

Acts as an advocate for veterans within the hospital system, wider VA, and community services whenever necessary. Participates with other members of the treatment team in attending weekly treatment team meetings, rounds, community meetings, family conferences, and other endeavors as needed and appropriate to the role of social work. Attends all administrative and programmatic meetings. Other duties as assigned.

Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience. Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.

Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66.
