Key responsibilities include conducting street based and community outreach to engage homeless Veterans who are not currently receiving VA services; providing case management with a focus on the most vulnerable populations; and collaborating with community agencies and VA programs including HUD-VASH, Grant & Per Diem, and Veterans Justice Outreach to connect Veterans to permanent housing and supportive services.
The Social Worker/Case Manager will coordinate and provide evaluation and other services supporting the functional abilities of Veterans who are struggling with issues of homelessness. The Social Worker will work directly with Veterans within the various community sites and will provide case management services and partner with community agencies to link access to community-wide housing services for Veterans.
The Case Manager will be providing services such as psychosocial assessments, treatment planning, individual, group and family therapy, discharge planning and follow-up, crisis intervention, case management assistance, resource development, and coordination related to these services as clinically indicated.
The Social Worker will provide outreach services at the full extent of their relevant clinical privileges, credentials, scope of practice, elements of practice, certification, functional statement, position description, or other VHA or local facility approved documentation of competency. Participating in team performance improvement and sustainment activities to optimize team efficiency and outreach services for Veterans.
Communicating with all team members including Veterans and caregivers to convey clinically relevant information for the care of the Veteran. Performing community-based outreach activities to engage homeless Veterans. Enrolling and providing case management services as clinically indicated, with a special emphasis on serving those who are the most vulnerable, as directed by the VA medical facility Homeless Program Coordinator.
Collaborating with VA medical facility Health Eligibility Center (HEC) Enrollment and Eligibility staff to initiate verification of Veterans' eligibility and in assisting Veterans in completing eligibility procedures. Performs other related duties as assigned. Work Schedule:8:00-4:30pm Monday-Friday. Work hours are subject to change based on agency needs.

If the position is determined to be eligible, the selectee must meet specific eligibility requirements per VHA policy and submit an EDRP application within four months of appointment. Program approval, award amount (up to $200,000), and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants are ineligible to apply.
Applicants selected for an EDRP-eligible position will be notified in their conditional job offer. Pay: Competitive salary and regular salary increases When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade).
Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience. Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.
Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66.
