This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment.
Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.
The candidate is a Social Worker in the Residential Rehabilitation Treatment Program (RRTP) within VA Central Western Mass HCS. The RRTP program provides treatment tracks for PTSD, SUD, and Mental Health Disorders. The incumbent participates on a treatment team and provides a full range of social work services including: psychosocial assessments, advance directives, care planning and care coordination, individual, couples, group treatment, and discharge planning.
Duties include but are not limited to: Psychosocial assessments Participate in the admissions/screening process. Provide individual and group treatment for PTSD, Substance Use Disorders, and a range of other Mental Health Diagnosis. Coordinate treatment and communicate with treatment team members to facilitate continuity of care for veterans. Active participant in the treatment planning process with other disciplines.
Responsible for ensuring that discharge plans are executed in a manner that is timely and appropriate. Provides case management to veterans and families. Provides wellness/prevention education and facilitate patient/family support groups. Ability to provide orientation and coaching to new social workers and social work graduate students. Maintain data and statistical compilations to comply with Joint Commission, VA and medical center policy and procedures.
Assist patients and their significant others with coping and dealing with the loss and grief experiences in disability, terminal illness and death. Committee participation as requested. Other duties as assigned.
